The Hospice RN Case Manager reports to the Team Manager. He/she is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families. This position supports the Senior Living Community & facilities in Baltimore City.
Education: Diploma, ADN and/or BSN from an accredited institution
Licensures/Certifications: * Current Maryland State license as a Registered Nurse
Experience: Two years of nursing experience, or equivalen
