Nation-wide primary care organization geared towards the geriatric population seeking Primary Care Advanced Practice Providers for their Rhode Island locations.
Monday-Friday 8am-5pm schedule (no nights or weekends)
12-16 patients per day once fully ramped up (could start lower if new site)
Patient population primarily medicare (over age 65) but see medicaid too so some sites have up to 25% under age 65
Collaborative care team - 1 MD, APP, Scribe, MA, Shared RN/Social Work support
